St. Thomas Aquinas School Acknowledgement Forms	Please note: The following forms are required for all St. Thomas Aquinas School families and all six (6) signatures are required for: Agreement of parent/child(ren) to comply with the STA Student Handbook Internet/Technology User Agreement – must be signed before students are issued Chromebooks. Photo Release Walker Release School computers cannot be used until the form is submitted. Students will not be released to walk/bike without an adult until the release form is signed.
