St. Thomas Aquinas School Acknowledgement Forms
Please note:  The following forms are required for all St. Thomas Aquinas School families and all six (6) signatures are required for:

  • Archdiocese Health and Safety Plan – MUST BE SIGNED BEFORE STUDENTS START SCHOOL
  • Agreement of parent/child(ren) to comply with the STA Student Handbook
  • Internet/Technology User Agreement
  • Photo Release
  • Walker Release

School computers cannot be used until the form is submitted.

Students will not be released to walk/bike without an adult until the release form is signed.

  • Please share the order form link (below) with your regular customers to allow for contactless sales!
  • Once ordering is complete, please submit your total order online.
  • Deliver your order money to school on September 3, 2021!

Lunch Break Chick-Fil-A OR Pasta Monday, Hot Dog (early release Tuesday), Hoagies & Hops (all other Tuesdays) & Pizza Friday must be ordered online by August 20. Payment will be billed using SMART Tuition scheduled payments. To order past due date, please contact Sheila Brown.
